Patient's attending the Practice for Orthodontic treatment must agree to the following terms and conditions prior to undertaking treatment.

 I will attend all necessary appointments at the practice. Should I be unable to attend an appointment I will cancel it at least 24 hours prior to the due appointment.

I agree to keep my brace, teeth and gums clean. I understand that if I fail to adopt a suitable oral hygiene regime my brace will be removed.

I accept that certain foods such as caramel, chewy sweets and sticky toffees will break the brace and I will need to avoid them. Repetative breakage of the brace will necessitate discontinuation of treatment.

After the brace has been fitted I will need to attend as requested for adjustment of the brace.

A course of orthodontic treatment can take up to 3 years to complete. If I start treatment I undertake to complete the work, unless there are circustances for not doing so such as ill health or moving out of the area.

A patient's treatment will be terminated/they will have deemed to have broken their contract with us if:

They fail/cancel with less than 24 hours notice, without good reasons, 2 appointments in a 6 month period. The onus will be on the patient/parent/guardian to contact us to remake the appointment.

Fail to attend for 6 months following a failed or cancelled appointment.

They fracture up to 3 components, in a course of treatment, 3 times.

Having attended with 4 or more components missing (ie. half the appliance), they fracture the appliance again.

PLEASE ALSO UNDERSTAND THAT DURING THE COURSE OF ORTHODONTIC TREATMENT THERE MAY BE OCCASIONAL DISCOMFORT. IF YOU ARE NOT PREPARED FOR THIS DO NOT UNDERTAKE THE TREATMENT.
